We understand that you may feel nervous about your C1 ambulance medical examination. Our doctor is here to make the whole experience a breeze. Here's some handy advice to help make your medical go well:
If you obtained your ambulance license after the month of January in the year 1997, you'll need to jump through this hoop. Don't worry, though; it's all for the greater good of safe and responsible driving.
You complete this using a D4 form, and it's your golden ticket to ambulance driving glory. Whether you're in the C1 camp or rocking the C category, this form has got you covered. You can find it here.
If you do need an ambulance / paramedical medical, we provide these in all our locations including Richmond, Heathrow, Reading, Croydon and Dartford.
Good news, my friend! You can breathe a sigh of relief because you only have to do one initial medical exam before the ripe age of 45—unless, of course, there have been changes in your medical history. In that case, make sure to spill the beans to the DVLA and let them decide if you need a repeat medical.
Once you hit the fabulous age of 45, it's time to mark your calendar because you'll need to face the C1/ ambulance medical assessment every five years. Just think of it as a thrilling reunion with your friends at D4YOU.
And if you're lucky enough to celebrate the glorious age of 65, get ready for an annual cambulance medical / C1 medical showdown. Yep, you heard that right—every single year, my friend.
The medical will take approximately 15 minutes. Longer if you stop for a chat and a biscuit. The ambulance medical / C1 medical is completed using the D4 medical form, also known as a D4 Medical, or a Driver Medical.
Our charming doctors will test your eyesight, check your blood pressure, and throw some medical questions [and maybe a biscuit or two] your way.
Alright, let's break it down for you—ambulance medicals, C1 medicals, the D2 and D4 forms, and all that jazz.
When it comes to your ambulance/C1 medical, the star of the show is the D4 form. To get your hands on this, you have a couple of options. You can head on over to one of those participating post offices, where they'll gladly hand it over to you. Alternatively, you can embrace the digital age and download the form here.
Oh, and let's not forget about the D2 form. It's like the trusty sidekick to the D4 form. You take the lead and fill out the D2 form yourself.
Ah, the great voyage of the D2 and D4 forms! You'll be pleased to know that they have a one-way ticket to their final destination—the DVLA in Swansea. Godspeed, dear forms, as you make your way to the DVLA in Swansea.
According to the DVLA website, they've set their sights on getting back to you within a three-week timeframe once you've bravely submitted your form. If you have health issues or further checks need to be conducted, the process may take more than 3 weeks. To save yourself from a long wait wait, our friendly advice is to get that medical ball rolling early.
Ambulance drivers and paramedic drivers have a medical check known as the D4 medical, driver medical, paramedic medical, C1 medical or LCV medical. Confusing, huh? Simple as that!

NHS GPs are swamped and as such, may not have capacity to conduct D4 DVLA medicals. This is because D4 DVLA medicals are considered non NHS work.
As per DVLA guidance, any GMC doctor with full registration can complete the medical.
Driver medical providers complete your DVLA medical, however don't play driver medical roulette. We've completed thousands of driver medicals, with expert knowledge and answers to all your burning questions.
D2 form: You fill it out to get your provisional license, like a driver's permission slip.
D4 form: It's the medical application form, a doctor's job. Lucky for you, we're experts at filling those out. Book an appointment and leave the paperwork to us!
A D4 medical usually takes about 15 minutes. Longer if you stop for a chat and biscuit.
Only a fully registered GMC registered doctor can complete this medical, and as per DVLA and it doesn't have to be your GP. But beware, amigo!
Choosing an inexperienced doctor or provider could lead to form fumbles and frustrating delays.
Some providers hire a mix of newbie and seasoned doctors for their clinics, leaving you guessing which you're gonna get. But fear not! Our GMC registered doctor is experienced in driver medicals.
Opticians can handle the vision test, but they can't do the whole D4 form. Good news though, we've got you covered! Our price includes the vision assessment, so no need to hit up the optician separately. We'll handle the whole shebang for you!
Our competitive price is just £45, and we include the eye test too!
You've got options, my friend:
Not every post office, my friend. Only participating post offices carry them.
You've got a four-month deadline from the day you got your form filled out, my friend. If you miss it, the DVLA might give it the cold shoulder, and you'll have to go through the medical exam all over again.
Ambulance drivers and paramedic drivers have a medical check known as the D4 medical, driver medical, paramedic medical, C1 medical or LCV medical.
Confusing, huh? But they all mean the same thing: undergoing a medical examination which is then recorded and completed on a form called the D4 form. This is why it is sometimes called a D4 medical.
It will also sometimes be called a category C or C1 medical as this is the license category you need to drive the ambulance.
These terms are used interchangeably but the bottom line is the same: any paramedics or ambulance drivers need to complete a medical using the D4 form.
Our doctor will use this to complete your medical and then you send it off to the DVLA. Simple as that!
